If you do not supply us with the necessary information, we will not be able to take any action, therefore please fill out all fields completely and precisely. As an alternative, you can send a written notice by fax or email. You have to include the following in your written notice:

  6. A declaration attesting to the accuracy of the information in the notification and attesting, under penalty of perjury, to the complaining party's authority to act on behalf of the owner of the allegedly violated exclusive right.
